ATHLETIC FORMS:
Parents, please download the following forms for the athletic physical packet: Sports Physical Cover Sheet, AMHS Sports Information Form, EL- 2 and EL-3 forms, and AMHS Waiver Form. Besides these forms, a copy of your health insurance card must be added to the packet and turned in to Coach Mark Saxton in the main office.
