REGISTRATION PROCESS

THE FOLLOWING INFORMATION MUST BE ON FILE FOR REGISTRATION:
  • Entrance Exam (grade 9)
  • Registration Packet
  • Application Form
  • Teacher/Principal Recommendations
  • Immunization Form (State of Florida) DH 680
  • Physical Form (State of Florida) DH 3040
  • Tuition Agreement Form
  • Final Transcripts from previous grade level(s)
  • United States Birth Certificate or Immigration Documentation
IMMIGRATION DOCUMENTATION
Immigration Documentation – “After September 11, 2001, the Department of Homeland Security (BCIS) expects the schools to have complete and accurate information pertaining to any foreign student entering their school. We must make sure that our students have the appropriate status that allows them to attend school in the United States”. The academically accepted student must provide one of the following forms of documentation: I-94; Notice of Action Form; Resident Alien Card; Authorization for Parole; Employment Authorization Card. The Archdiocese of Miami is Authorized Under Federal Law to Enroll Non-Immigrant Alien Students and Issue I-20 Certificates in Order to Obtain ““F-1”” Status. Those students who have an F-1 status must notify the school when leaving the country and again upon their return (i.e. holiday, summer breaks). If you need assistance, please let the school know at registration. It is our responsibility to remind all families that it is a violation of the tourist B-1 or B-2 visa to attend classes while on a B-1 or B-2 visa, and doing so could result in a cancellation of that visa. If a family is considering changing a B-1 or B-2 Tourist Visa for an F-1 student visa, they should consult with an immigration lawyer to help understand the implications of making an application. Should the family decide to make this application, Archbishop McCarthy High School will assist a family in whatever way possible to apply for the student visa. It often takes several months for INS to process and approve such applications; during this period the student may not attend classes in the school.
IMMUNIZATION AND PHYSICAL FORMS
It is mandatory that all students have on file in the school office a current FORM DH 3040/FLORIDA DEPARTMENT OF HEALTH SCHOOL ENTRY HEALTH EXAM AND FORM DH 680/FLORIDA DEPARTMENT OF HEALTH CERTIFICATION OF IMMUNIZATION. These forms are obtained from student’s physician and must be received prior the student attending classes.
RESIDENCY REQUIREMENT
All students enrolled at AMHS must be in residence with a parent or legal guardian. The administration reserves the right to dismiss a student if he/she is not in residency with the parent or legal guardian.
